To process your request, we must verify your identity (or that of your authorized agent) by requesting information such as contact details, account information, and/or health information to confirm you are the person about whom we collected the information.
You may designate an authorized agent with written permission. We may require direct verification from you and may deny requests without proper authorization proof.
For children under 13, only a parent or legal guardian may submit requests. We may require a signed consent form under penalty of perjury or a phone call to trained personnel for verification.
You may make a request to know or delete twice within a 12-month period.
Response Timing and Format We will confirm receipt of your request within 10 business days and provide details on processing, including verification and expected response timeline.
Changes to Our Privacy Notice We may update this Notice at any time. We will not use previously collected personal information for materially different purposes without your explicit consent.
